Liability Waiver

I hereby agree to indemnify and hold harmless the Humane Society of Southern Arizona, Inc., it's employees, agents and Board of Directors, from any and all liability arising out of or in consequence of, or injury sustained as a result of, any activity connected with myself or my child(ren) volunteering for the Humane Society of Southern Arizona. I also agree that I will make no claims for wages or compensation for the time in which I am performing volunteer functions.

I understand that photographs and videos taken of volunteers may be used in HSSA promotional materials. Checking the "I agree" box below constitutes consent to use any future photographs or videos in which I am featured.
